Biodiversity evaluation sheet: clearance and conversion (DOCX 65.2 KB) This evaluation sheet will ensure you consider Forest Practices Code provisions for biodiversity values when developing an FPP for clearance and conversion operations.

Biodiversity evaluation sheet: plantation operations (DOCX 57.4 KB) This evaluation sheet is designed to make sure you take Forest Practices Code provisions for biodiversity values relevant to plantation operations (roading, harvesting and reforestation) into account when preparing an FPP. (This includes operations which involve conversion of plantations to pasture.)

Biodiversity evaluation sheet: small scale plantation and plantation thinning (DOCX 40.2 KB) This evaluation sheet will ensure you consider Forest Practices Code provisions for biodiversity values relevant to plantation thinning operations; or harvesting in established plantations (where the total plantation area is less than 10 hectares)

Earth sciences and cultural heritage

Earth sciences and cultural heritage evaluation sheet  (DOCX 58.4 KB) This evaluation sheet will ensure you consider Forest Practices Code provisions for earth science and cultural heritage values when developing an FPP.

Earth sciences and cultural heritage evaluation sheet for small FPPs  (DOCX 102.4 KB) This is a special values evaluation sheet for FPPs of less than 10 ha.

Historical site recording form (DOCX 40.3 KB) Use this form for recording  historical cultural heritage sites.

Aboriginal heritage field assessment form

All actual or suspected sites (e.g. artefacts, unusual rock flakes, overhangs showing signs of human habitation, middens, scarred trees) should be notified using the AH Site Recording Form. This form should be sent to the  Cultural Heritage Manager at the FPA for checking and recording of site data on Conserve. The FPA will ensure that the form is sent to Aboriginal Heritage Tasmania so that site data can be recorded on the TASI database. Attach photographs for FPA and AHT records - forward these as separate jpg or tif  files.
